Specialist Focused Shockwave Therapy for Metatarsalgia

Specialist clinical icon for metatarsalgia identifying the metatarsal heads and forefoot joints as the primary target for focused shockwave treatment

Do you feel like you are walking on a pebble, or is there a persistent burning ache in the ball of your foot?

Our specialist clinic in Richmond provides advanced focused shockwave therapy (fSWT) for metatarsalgia, helping clients across the UK resolve forefoot pain and return to comfortable, active lives.

Metatarsalgia is an inflammatory condition affecting the metatarsal bones in the forefoot. It is often caused by repetitive high-impact activities, poorly fitting footwear, or changes in foot biomechanics. While rest and orthotics are commonly used first-line treatments, focused shockwave therapy provides an intervention that helps settle deep-seated inflammation and supports repair of overloaded soft tissues in the ball of the foot.

What Is Metatarsalgia?

The ball of your foot is a complex area where your long metatarsal bones meet your toes. These bones bear the majority of your weight during the "push-off" phase of walking or running.

When this area is overloaded, the surrounding joint capsules, ligaments, and even the bones themselves can become inflamed (bone marrow oedema). The sole of the foot has thick, protective skin, which can make deeper structures harder to reach. Focused shockwave allows us to precisely target the metatarsal heads.

Differential Diagnosis: Identifying Ball of Foot Pain

Metatarsalgia can be mistaken for other conditions: To ensure the correct treatment protocol, we need to distinguish it from:

  • Gout: Sudden, intense swelling and redness, usually at the base of the big toe.
  • Morton’s Neuroma: Specifically involves nerve thickening between the toes.
  • Stress Fractures: Bone-specific pain that is tender to the touch.

Treatment Protocol for Metatarsalgia

We have been treating metatarsalgia for clients locally in Richmond, across London, and throughout the UK since 2017, as such we create tailored treatment plans based on our experience, your symptoms and your recovery goals.

Typical Course

  • Starts with 5–8 sessions, usually once per week.
  • The exact number of sessions will depend on the severity of your condition and response to treatment.

Intensity And Frequency

  • Shockwave intensity and frequency are adjusted to your pain tolerance and the characteristics of the condition.

Minimal Downtime

  • Most clients can continue normal activities immediately.
  • Reduction in high-impact activity is advised during the first few sessions of the treatment plan.

Post-Treatment Sensation

  • Mild soreness or swelling may occur, usually resolving within a few days.

Each treatment plan is tailored to your condition. Learn more about what to expect from focused shockwave therapy and view a full breakdown of clinic fees on our prices page.

Benefits of Focused Shockwave Therapy for Metatarsalgia

Avoid Injections

Cortisone injections into the ball of the foot can lead to "fat pad atrophy" (thinning of the natural cushioning); shockwave is a regenerative alternative that preserves your foot’s natural padding.

Accelerated Healing

Focused shockwave therapy promotes faster healing of the area, often reducing overall recovery time compared to other treatments.

Precision Targeted Treatment

We identify exactly which metatarsal head (usually the 2nd or 3rd) is the source of the problem and focus the energy precisely to stimulate healing.

Effective Pain Relief

Many clients experience significant pain relief after just a few sessions, which can improve their ability to walk, run, and engage in exercise with less discomfort.

Improved Foot Function

As pain decreases and tissue health improves, clients often regain better function and strength in their feet, enabling them to return to their regular activities more confidently.

Fast Recovery

No crutches, no braces, and no downtime. You can continue your daily activities immediately.

When to Consider Focused Shockwave Therapy for Metatarsalgia

Stone in shoe sensation

If it feels like there is something stuck in your sock or shoe under the ball of your foot.

Burning or sharp pain

Especially when wearing thin-soled shoes or walking on hard surfaces.

Pain during push-off

If running or walking uphill causes a sharp reminder of your injury.

Swelling or tenderness

If the area under your toes is tender to direct pressure or appears slightly puffy.

Persistent pain

If metatarsal pain persists even after a period of rest and activity modification, particularly when pain interferes with daily activities or performance.

Resistance to standard treatments

If initial treatments such as orthotics and activity modification show little to no improvement.

Focused shockwave therapy applied early can stimulate healing and reduce inflammation in and around the meniscus, helping to manage pain and support recovery.

FAQs –Metatarsalgia & Focused Shockwave Therapy

Q: Is it painful to have shockwave on the ball of the foot?

A: The ball of the foot is a sensitive area, but because we use focused technology, the energy bypasses the skin and converges at the bone/joint level. Most clients describe it as a deep, manageable tingling.

Q: Can I wear my orthotics or pads over the course of the treatment?

A: Yes. We encourage you to keep using any "off-loading" pads or orthotics that make you comfortable. Our goal is to settle the inflammation so that you eventually won't rely on them as heavily.

Q: How many sessions will I need?

A: For forefoot inflammation, we typically recommend 5 to 8 sessions to ensure the inflammatory cycle is fully broken and the tissue has time to remodel.

Q: Does it work for Morton’s Neuroma too?

A: Metatarsalgia and Morton’s Neuroma often occur together. Focused shockwave is excellent for both, though for a Neuroma, we focus specifically on the nerve space between the bones.
